Volunteer Non-Disclosure (Confidentiality) Agreement

In many cases volunteers assist individuals and families with deeply personal medical information.  As you would expect, we treat this information with the utmost discretion.  Therefore, we ask all volunteers to sign a non-disclosure agreement.  All board and staff members sign this agreement.
